During your first physical therapy visit, you will undergo a comprehensive evaluation by one of our skilled physical therapists. Do I need a referral from a doctor to receive outpatient physical therapy?

That depends. Each state has laws that govern access to physical therapy services. Your insurance may also have rules regarding referrals for therapy. Typically, our physical therapists will be able to provide a screen or an evaluation without a physician referral but may need to work with your physician to provide treatment. Please contact your local LHMPTI clinic and our team will help to navigate the referral process, if one is needed.

For your first visit, please bring the following items so we can begin your care as smoothly as possible:

A photo ID

Comfortable clothing that allows you to move easily
Your smartphone or tablet, so we can help you set up the app that we use for your Home Exercise Program (HEP)

This app allows you to view your prescribed exercises and follow step-by-step instructions or videos so you can complete your program and stay on track between visits.
