LHM Physical Therapy Institute Hybrid Entity Designation

LHM Physical Therapy Institute, LLC (LHMPTI) complies with and is a covered entity under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations, both as amended from time to time (collectively, HIPAA).

Under HIPAA, a covered entity that conducts both HIPAA-covered and non-covered functions may elect to designate itself a hybrid entity as defined in 45 C.F.R. §§ 164.103 and 164.105. The covered entity must specifically identify its components that perform covered functions and designate these components as healthcare components. The HIPAA compliance obligations apply only to the designated healthcare components, and those units that perform support functions on their behalf.

Covered Functions

LHMPTI provides outpatient physical and occupational therapy services to its patients under a therapy plan of care. Therapy services are considered reasonable and necessary for the diagnosis or treatment of an illness, injury or condition. LHMPTI’s covered therapy services are listed here. All LHMPTI clinic locations provide HIPAA covered therapy services. The provision of such covered functions is supported by personnel in the LHMPTI administrative offices. LHMPTI designates its provision of therapy services and the associated administrative support as covered functions subject to HIPAA.

Non-Covered Functions

LHMPTI offers certain Health & Performance Services at each of its clinic locations. A complete list of those services is accessible here. LHMPTI’s Health & Performance Services and the associated administrative support are not subject to HIPAA and are considered non-covered functions.
