What to Expect at Your First Visit

Our LHMPTI team is looking forward to supporting you on your journey to healing. Our skilled physical therapists are committed to providing you with the highest quality of care in our state-of-the-art facilities. Here’s what you can expect once you schedule your first visit.

Shortly after scheduling your initial appointment, you will receive a welcome email containing a link to our secure third-party patient portal where you can complete your initial paperwork online prior to your clinic visit, at your own pace and in the comfort of your home.

If you do not wish to complete your paperwork at home, please arrive to your appointment 30 minutes early. Our friendly staff will help you register and complete the forms at our patient kiosk or by handwriting your responses.

As part of your intake paperwork, you will be asked to provide information about your medical history, interests, and goals.  Your therapist will use this information in discussions with you and to develop your individual therapy program. We also will ask for information about your health insurance so that we can verify your benefits prior to your visit. This allows us to counsel you regarding any out-of-pocket expenses and visit limitations to avoid any unexpected bills or delays in care.

For your first visit, we recommend that you wear comfortable, loose-fitting clothing that allows you to move easily. Your therapist will be evaluating your injured or involved body part, so make sure that your clothing will allow them to access the area to complete their exam. For your safety, please do not wear open-toe shoes, sandals, or flip flops. Depending on your condition, your therapist may request you bring specific clothing, such as athletic shoes or workout attire. They will provide you with any additional instructions regarding attire during your initial evaluation.

For your first appointment, please bring the following items:

 Drivers License
 Insurance Card(s)
 Medication list
 Insurance authorization, if applicable
 Any other health information that is relevant to your therapy visit (i.e., post-operative protocol, instructions from your physician, medical history, x-rays, test results, etc.)
 Referral from your physician or non-physician practitioner (i.e., nurse), if applicable
