LHM Physical Therapy Institute, LLC’s (LHMPTI) Health and Performance Services (HPS) are not subject to the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations, as amended from time to time (HIPAA). Although the Personal Information of HPS clients is not subject to HIPAA privacy protections, LHMPTI considers the privacy of its HPS clients to be of the utmost importance. This HPS Privacy Statement describes how LHMPTI may collect, use, disclose, and safeguard your information when you use the Health and Performance Services. Please review it carefully.
Information We Collect
We may collect information by which you may be personally identified, such as name, date of birth, postal address, email address, telephone number, payment information or any other identifier by which you may be contacted online or offline (“Personal Information”).

We use Personal Information that we collect about you or that you provide to us:

HPS Client Requests
As an HPS Client, you may:
- Request a copy of your HPS record, for which you may be charged a reasonable fee.
- Ask us to correct your HPS record. Although we will consider all such requests, we may decline if we have a reasonable basis to do so.
- Request that we not communicate your Personal Information to specific individuals. While we will strive to respect this request, we may do so if deemed reasonably necessary in response to a health or safety situation.
- Opt out of receiving promotional communications from us by following the unsubscribe instructions provided in those communications.
Changes to This Privacy Statement
We may update this Privacy Statement from time to time. We will notify you of any changes by posting the new statement on our website. Your continued use of HPS after any changes indicates your acceptance of the new Privacy Statement.
