During your first physical therapy visit, you will undergo a comprehensive evaluation by one of our skilled physical therapists. They will assess your condition, review your medical history, and discuss your goals and concerns. Based on this assessment, we will work with you to develop a personalized treatment plan tailored to your specific needs. Your program will focus on helping you achieve your goals and empowering you to get back to doing the things you love at home, at work, and in your community.

Do I need a referral from a doctor to receive outpatient physical therapy?

That depends. Each state has laws that govern access to physical therapy services. Your insurance may also have rules regarding referrals for therapy. Typically, our physical therapists will be able to provide a screen or an evaluation without a physician referral but may need to work with your physician to provide treatment. Please contact your local LHMPTI clinic and our team will help to navigate the referral process, if one is needed.

What should I bring to my first therapy visit?

For your first visit, please bring the following items so we can begin your care as smoothly as possible:

A photo ID
Your insurance card
Any referral or prescription from your physician or other healthcare provider (if applicable)
A list of current medications
Comfortable clothing that allows you to move easily
Your smartphone or tablet, so we can help you set up the app that we use for your Home Exercise Program (HEP)

This app allows you to view your prescribed exercises and follow step-by-step instructions or videos so you can complete your program and stay on track between visits.

What can I expect during my first visit?

What do I need before my first visit?

Once you are registered as a patient, you will be provided a secure login and password to the LHM Patient Portal. Please complete the assigned paperwork, medical history and functional questionnaire. Completing this information ahead of your appointment will help your therapist better understand the reason for your visit and allow them to tailor the evaluation to your specific needs.

Which insurance plans does your company participate with?

LHMPTI participates with most major insurance carriers, including Medicare and Medicaid. Coverage for physical therapy services varies based on your specific insurance plan and benefit package, and many plans include out-of-pocket costs such as copays, coinsurance, or deductibles.

Our team is happy to verify your insurance benefits prior to beginning treatment. We will do our best to help you understand your estimated out-of-pocket responsibility and avoid any unanticipated expenses.
