What to Expect During Your First Visit

During your first physical therapy visit, you will undergo a comprehensive evaluation by one of our skilled physical therapists. They will assess your condition, review your medical history, and discuss your goals and concerns. Based on this assessment, we will work with you to develop a personalized treatment plan tailored to your specific needs. Your program will focus on helping you achieve your goals and empowering you to get back to doing the things you love at home, at work, and in your community.

How do I pay for therapy services?

All out-of-pocket obligations (i.e., copay, coinsurance, deductible) are due at time of service. Payments can be made using a credit card, Health Savings Account (HSA) or by check. LHM Physical Therapy does not accept cash payments. If you receive a statement at the conclusion of therapy, you have several options to pay your outstanding balance. You can make a payment directly at the clinic, mail your payment with the statement coupon, or pay online through the Patient Portal by selecting “Pay My Bill” in the Patients drop-down menu.

Is an Authorization to Release form required before Medical Records will be provided?

Generally, yes. LHMPTI releases Medical Records in accordance with the applicable privacy laws, including HIPAA. There are limited situations where records may be provided without an authorization from the patient or their personal representative (e.g., if necessary for the patient's insurance company to process payment).

What do I need before my first visit?

Once you are registered as a patient, you will be provided a secure login and password to the LHM Patient Portal. Please complete the assigned paperwork, medical history and functional questionnaire. Completing this information ahead of your appointment will help your therapist better understand the reason for your visit and allow them to tailor the evaluation to your specific needs.

What is outpatient physical therapy?

Outpatient physical therapy is a type of rehabilitation and treatment provided in a clinic or outpatient facility, where patients receive therapy services without being admitted to a hospital. It involves various therapeutic techniques, exercises, and modalities to address musculoskeletal injuries, pain, mobility issues, and other conditions.

What should I do if I cannot make my appointment?

We understand that schedules can change. If you are unable to attend a scheduled appointment, please contact your clinic as soon as possible, preferably at least 24 hours in advance. Appointments that are not cancelled prior to the scheduled time will be considered a “no show” and are subject to a no-show or cancellation fee, which would be due at your next visit.

While we recognize that emergencies can occur, repeated missed appointments may impact your progress and our ability to provide care. Ongoing failure to attend scheduled visits may result in discontinuation of therapy and discharge from care, with appropriate notification to your referring provider or claims manager.
